Subject to the Rules and Regulations of the Fayetteville Public Works Commission, which, it is agreed, shall form a part of this contract, the undersigned hereby makes application to the Commission for installation of a sewer deduct meter at the premises specified below. Requirements: Application must be accompanied by a detailed drawing of the proposed meter location and showing the location of the primary meter, backflow prevention, and facility/equipment being served by the Deduct Meter.
The applicant agrees and guarantees to pay the Public Works Commission for all metering costs to include labor and meters at the premises for which this application is submitted in accordance with the rates, terms, conditions, rules, and regulations applicable to the service supplied hereunder, which shall upon date of this applicaiton, or at any time during the period the applicant is supplied service as provided for herein, currently in effect and as indicated in the Commission's rate schedule. The applicant further agrees that the express purpose of this request is for elimination of sewage charges for water supplied which does not enter the sewage system. Individual Liability for Joint Service: Two or more parties who join to make application for service shall be jointly and severally liable and shall be sent single periodic bills. This application is accompanied by an application fee in the amount of $100. This fee becomes the property of the Public Works Commission and in no case will be subject to refund.
